The highly qualified and compassionate staff of Jefferson Healthcare Home Health is available when you need health care brought to your home. Our team works closely with you and your physician to develop a plan of care following a recent illness, surgery, chronic disease management or change in health.

We have a comprehensive network of health care professionals including nurses, home health aides, medical social workers and physical, occupational and speech therapists, and a wound-care specialist to help patients and their caregivers become as self-sufficient and independent as possible. Our Home Health team members are specially trained, bringing a blend of extensive clinical experience, understanding and sensitivity to each patient’s needs.
