The highly qualified and compassionate staff of Jefferson Healthcare Home Health is available when you need health care brought to your home. Our team works closely with you and your physician to develop a plan of care following a recent illness, surgery, chronic disease management, or change in health.
We have a comprehensive network of health care professionals, including nurses, home health aides, medical social workers, physical, occupational, and speech therapists, and a wound-care specialist, to help patients and their caregivers become as self-sufficient and independent as possible. Our Home Health team members are specially trained, bringing a blend of extensive clinical experience, understanding, and sensitivity to each patient’s needs.
Phone: 360-385-0610 Location and Hours: The office located at 2500 W. Sims Way, Suite 300 is open 8:30 a.m to 5 p.m., Monday through Friday. Patients are seen daily, including on weekends. Nurses are on call 24 hours a day, seven days a week for our current patients.
