Planning for Discharge

When it is time for you to leave the hospital, our care team, including our case managers, will coordinate your discharge with you. Our case managers will consult with you and/or your family, your attending physician, nursing and other hospital therapies to coordinate and plan post-hospital care. The case managers are available to help you with arrangements for home care, home medical equipment, or long-term care needs.

Upon discharge, it is important that you understand your continued care program, including follow-up doctor visits and lifestyle recommendations. Some tips are listed below:

When possible, plan for needed home care prior to your hospitalization.
Arrange for friends and family to be available to assist you after discharge.
A friend or family member should be present when discharge instructions are explained.
Make sure your familyunderstands the discharge instructions issued by your health care team.
Make note of all contact phone numbers when you leave the hospital.

Check-out time

Patients are usually discharged in the morning. Please make arrangements for transportation home. Our discharge planners can assist you with your discharge needs, including transportation.
